Survey Participation Agreement

 
This Survey Research Participation Agreement (“Agreement”) is between Key2 Consulting, LLC (“Key2 Consulting”, “we”, “us”, “our”) and the survey participant (“Participant”, “you”, “your”)(collectively “the parties”).

In participating in this brief survey, you agree to the following:

1. Voluntary Participation. You understand that participating in this survey is entirely voluntary.

2. Data. All information that you share with us in this survey will be treated according to our Privacy Policy.

3. Only One Submission Per Work Email Address Allowed. Only submissions with a valid “work” email address (email contains company domain) will be eligible to win. Only one submission per work email address is allowed.
